What Is Endometriosis?
Endometriosis is a disease that affects women, usually between the age of 25 and 40. This disorder is one of the main causes of infertility among women.
It never appears before puberty and always ends at the time of menopause, due to a lack of ovarian hormones. The incidence of endometriosis is higher in infertile women, between 25-40 %, while in the general population endometriosis has an incidence of 3 – 10 %.
Endometriosis is characterized by the growth of uterine tissue outside of the endometrium. This uterine tissue grows inside of the myometrium or even outside of the uterus.
Internal Endometriosis – When the uterine tissue grows inside of the myometrium it is called Adenomyosis or Internal Endometriosis.
External Endometriosis – When the uterine tissue grows outside of the uterus it is called External Endometriosis. The uterine tissue can be found almost in every part and organ of the body, but it is more common in the ovaries, peritoneum, rectum, intestine, vagina, vulva, bladder, liver, etc.
Until today the real cause of pelvic endometriosis is not known. However, there are several theories that try to explain it. The most acceptable theories include:
Metaplasic theory
Implantation theory
Metastatic theory
Surgical scar implantation
Immune system disorder
To stay healthy, women should have a physical exam every year. Most of the time, the pelvic examination is a part of this annual exam. It’s also a time for the Obstetrician/Gynecologist to provide reassurance of normal anatomy and answer specific questions.
Pelvic Exam
A pelvic examination is a complete physical exam of a woman’s pelvic organs by a health professional. A pelvic exam helps a health professional evaluate:
Size of the vagina
Position of the vagina
Cervix
Uterus
Ovaries
It is an important part of preventive health care for all adult women. A pelvic exam is done to help detect:
Certain cancers in their early stages
Infections
Sexually transmitted infections (STIs)
Other reproductive system problems.
In some cases a pelvic ultrasound may be performed. A pelvic ultrasound is a noninvasive diagnostic pelvic exam that produces images that are used to assess organs and structures within the female pelvis.
What is a Laparoscopy?
The doctor can observe and perform a variety of procedures on a woman’s reproductive organs (the uterus, ovaries, and fallopian tubes). Since the surgeon uses instruments that fit into a narrow tube called a cannula, only a small incision is necessary.
When Laparoscopy Used?
Laparoscopy is used in the following surgical situations:

Tubal ligation, also known as having your tubes tied or tubal sterilization, is a type of permanent birth control. During a tubal ligation, the fallopian tubes are cut or blocked to permanently prevent pregnancy. A tubal ligation actually disrupts the movement of the egg to the uterus for fertilization and blocks sperm from traveling up the fallopian tubes to the egg.
This procedure does not affect your menstrual cycle. It can be done at any time, including after childbirth or in combination with another abdominal surgical procedure, such as a C-section. It is possible to reverse a tubal ligation, but reversal requires major surgery and isn’t always effective.
A Pap smear, also known as a Pap test, is a procedure used to test for cervical cancer in women. This screening procedure involves the collection of a sample of cells from a woman’s cervix, which is the lower narrow end of the uterus and is at the top of your vagina.
The collected sample of cells are then smeared or spread onto a microscope slide, so they can be sent to a lab for careful microscopic examination.
A Pap smear is a simple, quick and relatively painless screening test. Detecting cervical cancer early with a Pap smear gives you a greater chance at a cure.
A Pap test can also detect changes in your cervical cells that suggest cancer may develop in the future. Detecting these abnormal cells early with a Pap smear is your first step in halting the possible development of cervical cancer.
Pap smear has saved the lives of many women. Pap testing proved effective at detecting precancerous lesions, which represent early and still very treatable indicators of cervical cancer risk.
It’s the use of the Da Vinci robotic surgical system. Robot assisted surgery is an effective, minimally invasive alternative to both traditional surgery and Laparoscopy. Through tiny incisions (about 1-2cm), this robotic system can control every aspect of the surgery.
A computer console that contains hand controls is used by your surgeon to manipulate the machine’s robotic arms, wrists & other micro instruments. This robotic system provides a range of motion and agility that can far exceed human ability.
Pelvic floor reconstruction is a group of various surgical procedures used to treat pelvic organ prolapse, a condition that occurs when the muscles of the pelvic floor are weakened or damaged, often due to childbirth.

The pelvic floor includes the muscles, ligaments, connective tissue and nerves that support and control your bladder, uterus, vagina and rectum.
The pelvic floor holds the structures in your pelvis in place. Your pelvic floor is a very complex layer and usually functions very well, but it can be damaged if too much stress is placed upon it.
In particular, childbirth and the tremendous forces of labor with the dilation of your pelvic floor to allow for the passage of a baby and the huge pressures exerted during the pushing phase of labor all act to weaken, damage, or break your supporting tissues.
Hymenotomy
A hymenotomy is a minor hymen surgery procedure that involves removing part or all of a woman’s hymen. It is done for a variety of reasons, most of which have to do with the malformation of the hymen. Some women have this surgery to increase comfort during intercourse, while others need it for health reasons.
LEEP is short for Loop Electrosurgical Excision Procedure. The procedure uses a fine wire loop heated by an electric current to remove tissue & cells from the cervix. The hot loop acts as a surgical knife.
This procedure is used as part of the diagnosis & treatment of abnormal or cancerous conditions in women.
LEEP operation uses a wire loop that carries a low-voltage electrical current to cut away a thin layer of abnormal tissue.

It’s a simple 10-15 minute procedure that closely examines your cervix, vagina and vulva for signs of disease. It’s painless & performed right here in your gynecologist’s office.
Why it’s done?
Colposcopy can be used to diagnose things such as:
Cervical cancer
Vulvar cancer
Vaginal cancer
Genital warts
Inflammation of the cervix and Precancerous changes in the tissue of both the vagina and cervix
During the initial evaluation, a medical history is obtained, including:
Gravidity (number of prior pregnancies)
Parity (number of prior deliveries)
Last menstrual period
Contraception use
Prior abnormal pap smear results
Allergies
Significant past medical history, other medications
Prior cervical procedures
In some cases, a pregnancy test may be performed before the procedure. The patient must then sign a consent form.
Cervical cryosurgery or cryotherapy is a gynecological treatment that freezes a section of a woman’s cervix also used to treat some sexually transmitted diseases (STDs) like genital warts.
Cryotherapy uses special instruments called cryo probes that deliver liquid nitrogen. It is most often done to destroy abnormal cervical cells that are precancerous, which your doctor will refer to as cervical dysplasia & is over 85% effective.
Cryosurgery is usually done after a colposcopy confirms the presence of any abnormal cervical cells using Cervical Biopsy procedure. Cryotherapy is also used for the treatment of cervicitis or inflammation of the cervix & abnormal bleeding.
Cryotherapy is not a treatment for cervical cancer.
A pelvic ultrasound allows quick visualization of the female pelvic organs and structures including:
Uterus
Cervix
Vagina
Fallopian tubes
Ovaries
Ultrasound uses a transducer that sends out ultrasound waves at a frequency too high to be heard. The ultrasound transducer is placed on the skin, and the ultrasound waves move through the body to the organs and structures within.
The sound waves bounce off the organs like an echo and return to the transducer. The transducer processes the reflected waves, which are then converted by a computer into an image of the organs or tissues being examined.
The sound waves travel at different speeds depending on the type of tissue encountered – fastest through bone tissue and slowest through air.
The speed at which the sound waves are returned to the transducer, as well as how much of the sound wave returns, is translated by the transducer as different types of tissue.
